The Shamanka Trail Reservation Form

TOUR AGREEMENT: Reservations are now open for our September / October, 2010 journey (delete where applicable).  A deposit of $700.00 US Dollars secures your reservation; we request that you wire-transfer funds to our account.  Space is limited on a first-come, first-served basis. Final payment and required documentation are required in full 60 (sixty) days prior to departure. All cancellations must be made in writing, and no refunds will be made within 60 days of departure. Your initial deposit is non-refundable, unless insured with Trip Cancellation Insurance. We encourage the purchase of Travel Insurance for any non-refundable portion of your reservation. This signed Tour Agreement must accompany your deposit.  Please reconfirm all reservations before sending deposit in order to confirm that space is still available.

By acceptance of the following terms, the tour attendee agrees that The Shamanka Trail and its guides/presenters shall not be or become liable or responsible for any delays, delayed departure, or arrival, loss, death, damage or injury to person or property or accident, mechanical defect, failure or negligence of any nature, howsoever caused in connection with accommodations, transportation, or other services or for any substitution of hotels, or of common carrier, with or without notice, or for any additional expenses occasioned thereby. No revisions of the printed itinerary or its included features are anticipated; however, the right is reserved to make any changes, with or without notice that might become necessary, with the mutual understanding that any additional expenses will be paid by the individual passenger. By forwarding final payment the participant certifies that she/he has no physical, mental, or other condition of disability that would create a hazard for her/him or other passengers, and accepts the terms of The Shamanka Trail journey as advertised.

*Double occupancy rates are based on two persons sharing a room together; single supplement is available.   If you are travelling alone and need a roommate, we will attempt to pair you with someone; however, this is not guaranteed and if no roommate is available, a single supplement may have to be purchased.  To hold the current published rate, a Reservation with deposit is required, along with this signed form.  Rate does not include international airfare.
